3. Your phone number
4. The person your nominating's phone number
5. The person you are nominating (unless its you of course! :)
6. What type of illness or disability your/their child has.
7. How old is the child?
8. In a paragraph or two write why you want to nominate this family for a session and if its for you, why do you want a session?
